Case Management

SWAAD has trained case managers that are familiar with the community and its long term care services.  They are here to provide an array of options regarding available services and support for your long term care needs.

Our case managers are able to provide one-time assistance as well as ongoing support to assist vulnerable, aging and disabled adults in all five counties that we serve.

Case managers are available in the following programs and services*:
