You’re invited to join the End of Life Coalition of Southwest Washington and Honoring Choices Pacific Northwest community education presentation, Advanced Care Planning, on Wednesday, May 15, 2019 from 6:00pm-8:00pm at PeaceHealth Southwest Education Center. Documents that you need in order to complete your own plan will be provided at the presentation.
